Pacific Foundation for Medical Care: $5,000 to support development of the Family Medicine Residency Collaborative, a group of medical and healthcare organizations who are dedicated to maintaining a strong collaborative partnership with the Santa Rosa Family Medicine Residency Program and to ensure a stong presence of primary care physicians for the community now and into the future.
First 5 Sonoma County: $5,000 for literacy promotion in health care. In collaboration with the Santa Rosa Family Medicine Residency Program and Southwest Community Health Center, a reading corner and age appropriate books will be provided to pediatric patients in order to promote literacy and to screen for developmental milestones.
David Vanderryn Community Outreach Project: $20,000 in memory of David Vanderryn who died as a third-year resident and who was dedicated to quality medical care for the underserved. The Outreach Project will fund a pilot project for improving the scope of care and quality of care for patients through home visits, telephone support, and coordination with other community health services. The goal of the pilot project is to demonstrate that a team of Community Healthcare Workers, working with a resident-faculty team and other residency health partners, can make a large difference in health care outcomes and can do it extremely cost-effectively.
